Getting Started: A password is required to open these documents please read and follow the directions in their entirety.

Getting Started with TCLC Mississippi Behavior Clinic Outreach and consultation is easy.

First you must complete an official intake. This is how we first learn of your needs and it allows us to begin the process of identifying your needs and setting up an initial appointment.

You may call our offices to start an intake at (601) 899-5900 our staff will guide you through the balance of the process. You will need to let the individual handling your intake know if you are requesting an intensive home program provided by Outreach and Consultation or if you are a school district or other organization requesting assistance.

You will need to complete some initial paperwork in addition to conducting an intake interview.

For your convenience we have posted much of the documents you must complete here on the website.  If you are an individual seeking assistance in your home this paperwork is located in the area titled for individuals or home programs:, if you are a school district or other organization this paperwork appears on the left of this page under for school districts or organizations.

You will need to have Acrobat Reader installed on your computer to use these files. If you do not have Acrobat Reader you can download it free using the link that appears on this page.

 

In order to use these forms you will need a password. In order to obtain that password you will first need to complete the intake interview with our staff. Please call our offices to complete that intake interview soon.

Before the Initial Consultation:

Be sure to check the supplies page to determine the type of supplies you will need for the initial consultation. Unless your consultant makes changes to this list you will need all of these supplies on the first day of your consultation.
